Typical First Visit

• Please arrive 20-30 minutes prior to your scheduled appointment to complete the necessary paperwork, review your insurance benefits and change into comfortable clothes as necessary.
• Your first physical therapy session will include a physical therapy evaluation which involves taking measurements and performing tests to determine how physical therapy can help improve your symptoms. Your physical therapist will then analyze these tests and measurements, develop a plan and explain how they plan to treat your symptoms.
• Your first appointment and each subsequent session will be 60 minutes depending on your diagnosis. You will always be treated 1-on-1 with your physical therapist for your entire treatment duration.
• Please bring the following to your first appointment:
  • Signed prescription from your MD
  • Insurance Card & Photo ID
  • Completed New Patient Form (PDF) as able
  • Comfortable Athletic cloths and shoes

** Note: If the patient is a minor, a patient or legal guardian must be present at the first appointment**

    • LGOST accepts any prescription form from an MD, even if it is for a different PT provider. Or download our form for your MD to fill out.
• Insurance
  • As a courtesy to all of our patients, we will verify your insurance coverage and review your coverage with you prior to your first physical therapy appointment.
  • We accept most PPOs, Private (Cash) Pay, Medicare and Worker’s Compensation. For detailed information on how your insurance will cover your physical therapy appointments please contact us at (408) 358-1460.
